Yardley Readmissions Analyzer™ is a workstation based tool with flexible logic intended mainly for TRENDSTAR customers (since readmission analysis is handled by HPM's Encounter Analysis capability).  You can decide the time frames and what consitutes a related readmission.  The solution offers several standard reports and exports a file for use in TRENDSTAR or other tools.
